Members who are already enrolled in the UFT Welfare Fund can use this expedited change of status process to:
- Make changes to your name
- Update your mailing address
- Update your family profile including dependents and beneficiaries

Here are some tips to help you through this process:
When requesting a Change of Status online, you must provide your Date of Birth and either your Social Security Number or UFT Welfare Fund Alternate ID Number.
After you have completed all applicable pages of your Change of Status (COS) online, a summary of your request will display. You will have the opportunity to review it and make any necessary changes, if needed.
Please remember to send us a copy of your cover page along with supporting documentation in order for us to update your records.
Your Email Address:
The email address displayed throughout the COS pages should be your non-DOE email address, as it will enable the UFT Welfare Fund to communicate with you quickly. Your email address is also your login username.
To change your email address, you must update your profile located as a link on the top right corner next to the search button, or click here.
If you change the email address on your profile, all UFT and UFT Welfare Fund generated email will be sent to the new email address. Also, the new email address will become your login username for access to the UFT website.
Change Member Name:
Provide current name information if different due to marriage, divorce, separation, death or legal name change.
The Welfare Fund requires supporting documentation to process a name change request. At the completion of your online COS request, please print the COS Cover Letter and mail it to us along with a photocopy of the applicable document from the following list:
- Marriage Certificate
- Divorce Decree
- Legal Separation Document
- Death Certificate
- Petition for Name Change
Change Member Mailing Address:
Provide current mailing address information and indicate if it is a permanent or temporary (seasonal) address. If applicable, include your apartment number with your home address.
Add a Spouse / Domestic Partner:
The Welfare Fund requires supporting documentation to process a request to add a spouse or domestic partner. At the completion of your online COS request, please print the COS Cover Letter and mail it to us along with the applicable document(s) from the following list:
- If married, send a photocopy of your Marriage Certificate.
- If domestic partnership:
- If you reside in NYC, send the original Approval Letter issued by the NYC Employee Benefits Program and/or Domestic Partner Registration Certificate.
- If you reside outside NYC, send the original Approval Letter issued by the NYC Employee Benefits Program and/or Domestic Partner Registration Certificate and alternative Affidavit of Domestic Partnership.
- Please note, for Domestic Partnerships only, if you are only adding your domestic partner to the Welfare Fund and not to your City Health Benefits, you must also submit a “Declaration of Financial Interdependence” form.
Drop a Spouse / Domestic Partner:
The Welfare Fund requires supporting documentation to process a request to drop a spouse or domestic partner. At the completion of your online COS request, please print the COS Cover Letter and mail it to us along with the applicable document(s) from the following list:
- Divorce Decree
- Legal Separation Document
- Affidavit of Domestic Partnership Termination
- Death Certificate
Change in Dependent Information:
The Welfare Fund requires supporting documentation to process a request to add or drop a dependent child(ren). You can mail supporting documents to us or upload the documents using our online system.
Applicable document(s) for dependents:
- If adding a dependent child(ren),
- Photocopy of Birth Certificate and/or Guardianship Papers, whichever is applicable for each child.
- For each dependent child between the ages of 19 and 23 you must include a UFT Welfare Fund Dependent Student Certification Form if applicable.
- For each disabled dependent child over the age of 19, if applicable, you must include a UFT Welfare Fund Disabled Dependent Child Affidavit (DDCA) form, along with the acceptance of the disability letter from your basic health carrier.
- If dropping dependent child(ren), please attach appropriate documentation
- A notarized letter stating reason and date of event.
- Death Certificate.
Update Beneficiary Information:
Please enter all your beneficiaries, as this will update your existing designations.
- If more than one beneficiary is being designated, please make sure you allocate a percentage for each beneficiary. The total must add up to 100 percent.
- You may designate your estate instead of individuals.
IMPORTANT: If your Change of Status request involves any of the following changes, it cannot be submitted online:
- Profile change (i.e. name, date of birth, etc.) of spouse/domestic partner already enrolled in the Welfare Fund.
- Profile change (i.e. name, date of birth, etc.) of dependent child(ren) already enrolled in the Welfare Fund.
- Change marital status (i.e. from spouse to domestic partner or vice versa) of spouse/domestic partner already enrolled in the Welfare Fund.

Finalizing Your Changes:
Upon completion of your Change of Status (COS) online process, if supporting documentation is required by the Welfare Fund in order to complete your request, you will be asked to print a Welfare Fund Change of Status Cover Letter. You will need to submit the COS Cover Letter along with all supporting documentation, via mail, before the Welfare Fund can update your records.
